Agent-aware code review tool that traces each PR line back to the agent turn that produced it

Developers reviewing AI-agent-written code see the final diff but not which agent turn, prompt, or tool call produced each line. A review layer that captures agent sessions and annotates PR diffs with line-level provenance would make approvals and debugging much easier.
